Carers Project
What does the project provide?
The Carers Project aims to raise awareness about carers issues
and generate new referrals through to the existing support services
of the Hammersmith and Fulham Carers Centre and local NHS
services.
Who does the project reach?
The project targets (adult and young) hidden carers through,
primary care teams, GP practices and other voluntary organisations.
The carers are people who are caring for a relative or friend who
has a serious illness, disability, mental health issues or
substance misuse issues.
Unique selling point?
- Producing new and eye catching promotional material and
ensuring its circulation amongst relevant individuals, institutions
and social, educational and health care professionals.
- Facilitating awareness raising sessions for NHS staff, social
services and other related professionals
- Linking with GP practices to design user friendly referral
forms and lines of communication to ensure increasing numbers of
vulnerable hidden carers access support services
- The carers centre has historically been unable to make
meaningful links with GP practices. The HLC carers project has been
able to bring about new referrals and new partnerships.
