What Health Issues are you able to help
with?
Hammersmith and Fulham health trainers deal with many general
health issues including:
Smoking: Stopping smoking is difficult but we can provide the
advice, support and encouragement that you need. Whether you choose
to follow an NHS programme or go it alone we are here to help.
Alcohol and Substance abuse: If you would like support in reducing
your alcohol or drug use we can help. We can put you in touch with
professional organizations and meet with you regularly ourselves to
encourage and motivate you.
Physical Activity: Many of us need to become more active,
especially as we get older. There are lots of free and low-cost
activities available in the borough and we can help find the most
suitable and fun activities for you.
Emotional Well-Being: Life is hard and we all feel low from time to
time, however, sometimes it is more difficult to pick yourself up.
Our trainers offer support and advice on the small changes that you
can make to help yourself feel better.
Diet: We can advise on simple changes to your diet that will really
make a difference to your overall health. We offer advice on
shopping, cooking and eating out.
Who is the service open to?
Our service is open to any adult resident of Hammersmith and
Fulham, not just Shepherds Bush Housing Association residents.
What areas of Hammersmith and Fulham do the health trainers
cover?
Our team of health trainers provide a service that reaches across
the whole of Hammersmith and Fulham. We offer drop-in sessions and
appointments at locations in the North, Central and Southern areas
of the borough.
Is the service confidential?
Yes, any information that you tell us will be kept solely between
you and our health training team. We will always ask for your
permission before referring you to another service.
Where will meetings take place?
Your meeting will take place in an agreed community venue. We will
try and see you as locally as possible, and may be able to arrange
home visits in special circumstances.
Do I need a GP referral to access the
service?
No, we do accept GP referrals, however we will be running a number
of drop-in clinics and you can also call us to arrange an
appointment on 0208 846 6441.
